Note: This form must be completed to obtain an RMA#.

  1. Your Operating Manual contains a multitude of information to help you with your unit, please refer to it for operating questions and instructions. We also suggest reading the Troubleshooting section of the manual.
  2. Controllers and scan heads with the same serial nos. were built to work together. Pairing components from different systems may cause measurement errors and profiling problems.
  3. If we determine that it is necessary for your unit to be sent back to our facility for evaluation and repair, we will provide a Return Material Authorization (RMA) number for your unit. For quality purposes, RMA numbers are only good for 30 days.
  4. For questions on warranty for units purchased within the last 12 months, please refer to the Warranty Information sheet.
